1. Application for Health Insurance

  • Proof of Identity: National ID, driver’s license, passport, or any government-issued ID.

  • Proof of Address: Utility bill, rental agreement, or government-issued ID with address (as per insurer requirements).

  • Age Proof: Birth certificate, passport, school certificates, or Aadhaar card (varies by country).

  • Photographs: Passport-sized photographs of the applicant and any dependents to be included.

  • Employment Details: Employment verification letter, pay stubs, or any company-provided documents (if employer-sponsored).

 

2. Medical History and Health Details

  • Medical Reports: Recent medical reports, particularly for those over a certain age or with pre-existing conditions.

  • Family Medical History: Some policies require an outline of family history for genetic conditions.

  • Declaration of Pre-existing Conditions: Disclosure of any known health conditions for accurate underwriting and risk assessment.

 

3. Income Verification (if applicable)

  • Salary Slips: Past 2-3 months' pay stubs or employment verification.

  • Tax Returns: Previous year’s tax filing documents (useful for calculating premium subsidies or eligibility for certain plans).

  • Bank Statements: Can be required to prove affordability or eligibility for certain low-cost plans.

 

4. Employer-Sponsored Health Insurance

  • Employee ID: Company-provided identification.

  • Employer-provided Documents: Health insurance enrollment.
