  • Travel Itinerary: Details of the travel itinerary, including flight tickets, hotel reservations, and other bookings related to the trip. This confirms the duration and locations covered.

  • Medical History: Basic medical information, pre-existing condition declarations, or recent health checkups, depending on policy requirements. Insurers often need this to assess medical risk factors.

  • Destination Information: Proof of destinations and activities planned, especially for adventure activities or high-risk areas. Some policies may have exclusions based on destinations or activities.

  • Emergency Contact Information: A list of emergency contacts, including the traveler’s family members or close friends who can assist if needed.

 

2. Claims Documentation

For various types of claims (medical, lost luggage, trip cancellation), the following documents are generally required:

  • Claim Form: Completed claim form with the policy number, traveler’s details, and specific information about the claim.

  • Medical Claims:

    • Medical Reports: Diagnosis, treatment details, and medical report from the treating physician or hospital.
